If a friend Responded to an invite with and guest...how do we write out the place card?  At this time he doesn't have a name for the guest and the cards must be done.

Is it ok to put Ben Smith and Guest?

Since your friend didn't provide the name of his guest, you can either call and ask for a name or do as you suggest and write out two cards, one saying "Ben Smith" and the other simply saying "Guest."
